Free In-Home Consultation
To ensure we offer the best possible service, St. Francis Home Health Agency provides a Free In-Home Consultation. One of our care coordinators will meet with you and/or your loved one to properly assess your needs so that we can create a care plan specifically tailored to your requirements.
The Consultation
We will set up a care plan based on your needs and requirements of our care staff.
Our Consultation will include collecting:
- Your General Information
- Specific diagnosis
- Required Instructions and level of assistance needed from our staff
- Medications being used, frequency and the dosages
- Hours of Service required
- Notation of daily duties you will require of our caregivers or nurses
- We will also be more than willing to provide any answers to any concerns, or questions regarding the care for you or your loved one, our rates, our caregivers/nurses, etc.
